Your Plan For Health Website Launch!

Your Plan For Health

Click here to view the new site!

Faculty and staff, you now have a great new resource for maintaining health and well-being! Your Plan for Health has just recently launched their new website, designed to help YOU as a faculty or staff member here at Ohio State reach your fitness and health goals.

What to look for on the new site:

New Personal Health & Well-Being Assessment (PHA)
- The new PHA was designed to take a holistic look at health AND overall well-being;
- It has 146 questions and takes about 20 minutes to complete;
- Biometric health values obtained during a free, on-campus Biometric Health Screening are still automatically uploaded by a Health Plan nurse into the user’s PHA in the portal;
- Health values obtained during a visit with a personal physician must still be entered into the PHA by the user.

Set your goals, track your progress
- The new site provides a unique, interactive, and personalized experience;
- Compete with friends in healthy challenges established by Ohio State, or create your own;
- Set personal health goals to address areas in which you’d like to improve;
- It’s easy to track and monitor your progress – the system can sync up with various fitness devices (e.g., FitBit, Nike+, FitLinxx) and mobile apps (e.g., RunKeeper);
- Mobile device integration and responsive design – you can record your healthy actions on your desktop, tablet, or smartphone and have the same great experience.

Earn points and rewards in the new incentive program
A new design encourages ongoing engagement through gamification;
Earn rewards as you progress through three levels:
- Get In: Complete your PHA for a $360 PHA credit (faculty/staff) or a $120 PHA credit (spouses/same sex domestic partners, starting 2014) and join at least one Challenge (500 points).
- Get Going: Have a primary care physician, be (or work toward being) tobacco free, and get started with Challenges for a $75 Amazon Gift Card (2,000 points, including points accumulated in previous level).
- Get Results: Engage in Challenges for a fitness device and entry into a grand-prize drawing (3,000 total points, including points accumulated in previous levels).

Get Engaged
- Participate in community forums;
- Nominate yourself or other faculty and staff to be featured as a YP4H Champion;
- Submit a wellness-focused to the Health & Wellness Events Calendar;
- Share Your Story for a chance to be featured in the YP4H Spotlight section;
- Engage with our new social media platforms, including Facebook, Twitter, and Pinterest.
